Obtaining a Copy of a Birth Certificate in Las Vegas, Nevada

Losing your birth certificate can be a very big hassle. This is especially true if you want to get a passport or an employer wants to see identification and you don't have a Social Security card. In Clark County, home to the city of Las Vegas, you can easily obtain a copy of your birth certificate on the county website, through the mail, via fax or in person.

Step 1

Log onto the Southern Nevada Health District Birth/Death Certificates page (see Resources below).

Step 2

Send a request through the mail. Click the first "birth certificate Application" link found underneath the "Birth Certificate Application Requests" heading. Complete and print that form. Send that, a money order made payable to SNHD and a copy of your photo ID to: Southern Nevada Health District--Vital Records Office P.O. Box 3902--625 Shadow Lane Las Vegas, NV 89127

Step 3

Get a copy of your birth certificate via fax. Click the second "Birth Certificate Application" link found on the page. Fill out and print this form. When finished, fax it to (702) 383-4095. Make sure the credit card information has been filled out properly, or else your order will not be processed.

Step 4

Click the "Online Birth Certificate Application" link to request a copy of your birth certificate over the Internet. Along with a shipping address for the birth certificate, you must enter billing information for a Visa or Mastercard. You must also scan your photo ID onto your computer and upload it into the system. Otherwise, your order will not go through.

Step 5

Obtain a copy of your birth certificate in person. For Las Vegas residents, visit the East Las Vegas Public Health Center. This is located at: 560 N. Nellis Boulevard Suite E12 Las Vegas, NV 89110
